ABSTRACT

Abstract Background Levodopa-induced dyskinesia (LID) is a common motor complication of levodopa therapy in patients with Parkinson's disease (PD). Doxycycline is a widely used and inexpensive tetracycline with anti-inflammatory properties. Objective To evaluate the efficacy and safety of doxycycline in patients with PD and LID. Methods This was an open-label, uncontrolled, single-arm, single-center, phase 2 proof-of-concept study in patients with PD with functional impact of dyskinesia, which used levodopa three times daily, in a movement disorders clinic in Brazil. Participants were treated with doxycycline 200 mg/day for 12 weeks, with evaluations at baseline, week4, and week 12 of treatment. The primary outcome measure was the change from baseline in the Unified Dyskinesia Rating Scale (UDysRS) total score at week 12, evaluated by two blinded raters. Key secondary outcomes measures were OFF time and ON time with troublesome dyskinesia in the PD home diary. Results Eight patients with PD were treated and evaluated. Doxycycline 200 mg/day reduced the UDysRS total score at week 12, compared with baseline (Friedman χ2 = 9.6; p = 0.008). Further, doxycycline reduced the ON time with troublesome dyskinesia (Friedman χ2 = 10.8; p = 0.004) without worsening parkinsonism. There were no severe adverse events, and dyspepsia was the commonest event. Conclusion In this preliminary, open-label and uncontrolled trial, doxycycline was effective in reducing LID and safe after a 12-week treatment. Further well-designed placebo-controlled clinical trials with a longer duration and a larger number of participants are needed. Resumo Antecedentes A discinesia induzida por levodopa (DIL) é uma complicação motora comum da terapia com levodopa em pacientes com doença de Parkinson (DP). A doxiciclina é uma tetraciclina amplamente usada e barata, com propriedade anti-inflamatória. Objetivo Avaliar a eficácia e segurança da doxiciclina em pacientes com DP e DIL. Métodos Este foi um estudo aberto, não-controlado, de braço único, monocêntrico, fase 2 e de prova de conceito, em pacientes com DP e impacto funcional das discinesias, que usavam levodopa três vezes ao dia, em um ambulatório de distúrbios de movimento no Brasil. Os participantes foram tratados com doxiciclina 200 mg/dia por 12 semanas, com avaliações na base, na semana 4 e na semana 12 do tratamento. A medida de desfecho primário foi a mudança no escore total da Unified Dyskinesia Rating Scale (UDysRS) da base à semana 12, avaliada por dois avaliadores cegos. As medidas-chave de desfecho secundário fora o tempo em OFF e tempo em ON com discinesia problemática. Resultados Oito pacientes com DP foram tratados e avaliados. A doxiciclina 200 mg/dia reduziu o escore total da UDysRS na semana 12, comparado com a avaliação inicial (χ2 de Friedman = 9.6; p = 0.008). Além disso, a doxiciclina reduziu o tempo em ON com discinesia problemática (χ2 de Friedman = 10.8; p = 0.004) sem piorar o parkinsonismo. Não houve eventos adversos graves, e dispepsia foi o evento mais comum. Conclusão No presente estudo preliminar, aberto e não-controlado, a doxiciclina foi eficaz em reduzir as DIL e segura após tratamento por 12 semanas. Estudos clínicos bem-desenhados e placebo-controlados adicionais, com duração mais longa e maior número de participantes, são necessários.

RESUMO A ceratopigmentação teve seu primeiro registro pelo filósofo Galeno há muitos séculos como uma estratégia utilizada para o tratamento estético de pacientes com leucomas. As córneas com leucoma são patológicas e, muitas vezes, intolerantes a lentes de contato cosméticas ou próteses oculares, sendo comum a queixa de desconforto excessivo, proporcionado pela superfície corneana irregular. Assim, a ceratopigmentação é uma alternativa para a melhora estética de pacientes com opacidades corneanas. Descrevemos o caso de um paciente do sexo masculino, 39 anos, que apresentou despigmentação precoce em caso de ceratopigmentação associado a quadro de ceratite herpética necrotizante. O paciente foi submetido ao tratamento com aciclovir 2g ao dia e doxiciclina 200mg ao dia, evoluindo com melhora do quadro clínico, apesar da má adesão medicamentosa.


ABSTRACT Keratopigmentation was first recorded many centuries ago by the philosopher Galeno, as a strategy used for the aesthetic treatment of patients with leukomas. Corneas with leucoma are pathological and often intolerant of cosmetic contact lenses or ocular prostheses, with complaints of excessive discomfort provided by the irregular corneal surface being common. Therefore, keratopigmentation is an alternative for the aesthetic improvement of patients with corneal opacities. We describe the case of a 39-year old male patient, who presented early depigmentation in a case of keratopigmentation associated with necrotizing herpetic keratitis. The patient was treated with Acyclovir 2g/day and Doxycycline 200mg/day, evolving with clinical improvement, despite poor medication adherence.

ABSTRACT

The severe acute respiratory syndrome coronavirus 2 genetic variation must be closely monitored. Viral transmission can inevitably result from mutations in the viral genome and functional proteins that aid in the virus's adaptation to the host. This study aimed to look for mutations in the E protein and see how they affected the ligands' molecular docking. Mathematical saturation mutagenesis and other informatics techniques were used. Fourteen severe acute respiratory syndrome coronavirus 2 isolates from Iraq were selected. Doxycycline and rutin were chosen as ligands. In four strains of severe acute respiratory syndrome coronavirus 2, the N15Y mutation was detected in the envelope protein. Depending on the calculation of the amount of energy of the atoms, this mutation is critical in modifying the shape of the protein as well as increasing protein stability. In the single-chain mutation, one pocket was determined, while all pentamer chains had two pockets. The N15Y mutation altered the degree of doxycycline binding by affecting the residue of attachment of the ligands. It also altered the position of the rutin's attachment to the E protein, which has a clear impact on the virion particle(AU)


La variación genética del coronavirus 2 del síndrome respiratorio agudo severo debe ser monitoreada de cerca. La transmisión viral puede resultar inevitablemente de mutaciones en el genoma viral y proteínas funcionales que ayudan en la adaptación del virus al hospedero. Este estudio tuvo como objetivo buscar mutaciones en la proteína E y ver cómo afectaban el acoplamiento molecular de los ligandos. Se utilizó la mutagénesis de saturación matemática y otras técnicas informáticas. Se seleccionaron 14 aislamientos del coronavirus 2 del síndrome respiratorio agudo severo en Irak. Se eligieron doxiciclina y rutina como ligandos. En cuatro cepas del coronavirus 2 del síndrome respiratorio agudo severo, se detectó la mutación N15Y en la proteína de la envoltura. Dependiendo del cálculo de la cantidad de energía de los átomos, esta mutación es fundamental para modificar la forma de la proteína y aumentar la estabilidad de la proteína. En la mutación de cadena única, se determinó un bolsillo, mientras que todas las cadenas de pentámero tenían dos bolsillos. La mutación N15Y alteró el grado de unión de doxiciclina al afectar el residuo de unión de los ligandos. También alteró la posición de unión de la rutina a la proteína E, lo que tiene un claro impacto en la partícula del virión(AU)

ABSTRACT

Background: Tetracyclines, in particular doxycycline, are recommended for the treatment of patients with acute undifferentiated febrile illness (AUFI); however, real-world studies are scarce. Methods: This retrospective, multicenter, observational study reviewed electronic medical records (April 2018 to March 2021) of adult patients (outpatient and inpatient departments [OPD and IPD]) with AUFI, treated with doxycycline monotherapy (doxycycline group) or doxycycline in combination with other antimicrobials (combination therapy group), from 7 tertiary hospitals and clinics in India. Results: Overall, 473 patients were included; 73.8% and 26.2% patients were prescribed doxycycline alone or in combination with other antimicrobials, respectively. Defervescence was achieved in 65.6% and 57.3% patients, respectively at the second (8-14 days) follow-up visit. Clinical cure rate for symptomatic resolution varied between 89.6% and 100% in OPD settings. Time taken from treatment initiation to defervescence was 3.51 ± 3.16 days for the doxycycline group and 3.46 ± 3.07 days for the combination therapy group. Both groups showed improvements in body temperature in OPD settings (84.2% and 84.5%) as well as IPD settings (97.4% and 94.1%). Adverse events in OPD patients in both groups were nausea (7.8% and 8.7%), anorexia (1.6% and 33.0%) and dyspepsia (1.6% and 67.9%). Conclusion: Doxycycline appears to be a promising candidate for treating patients with AUFI due to its demonstrated real-world effectiveness and safety profile.

ABSTRACT

Background:Dengue (pronounced Dengee) fever is a painful, debilitating mosquito-borne disease caused by any one of four closely related dengue viruses. Aim of the study was to study radiological features of dengue fever in cases and control.Methods:Randomized, double blind, placebo controlled, parallel group study conducted among 100 seropositive dengue patients,18-60 years of age (fulfilling inclusion and exclusion criteria) at Lala Lajpat Rai hospital, Kanpur for December2019 October 2021. Selected patients were subjected to history, examination, necessary investigations and then were managed according to national vector borne disease control programme (NVBDCP) guidelines. A total of 100 serology proven dengue patients were included and followed up. Two groups (fifty in each) were allocated by simple first and then systematic random sampling. Case group was given doxycycline and control group was given placebo. Results:USG findings commonly seen in both the cases and control group were hepatomegaly, splenomegaly, ascites right pleural effusion, thickened GB wall and edema. In both the groups, pleural effusion was the most common complication.Conclusions: Ultrasound examinations detected plasma leakage in multiple body compartments around time of defervescence. Presence of plasma leakage in form of ascites and pleural effusion was more in control group as compared to case group seems doxycycline decrease the plasma leakage. Pleural effusion is most common finding ofplasma leakage.

ABSTRACT

Scrub typhus (ST) is a consistently underreported disease. The disease is spreading to newer areas, and an understanding of disease epidemiology is needed in the local Indian and current context. This study describes the demographic characteristics, monthly distribution, clinical and laboratory presentations, and treatment outcome of the ST cases recently identified. Case sheets of 15 ST patients diagnosed from January 2019 to December 2020 were analyzed. The majority of the patients were male. Eighty percent of the patients were from rural or suburban areas. Higher admission was observed from September to December. Fever (100%), skin rash (73.3%), body ache (53.3%), and vomiting (53.3%) were the most common clinical features. The onset of fever till the appearance of skin rash was 3 ± 1.2 days. The mean day from onset of symptoms till diagnosis was 6.8 ± 3.9 days. Eschar was found in only two patients. Nine (60%) patients already had complications at the time of admission. Most patients presented a laboratory picture of thrombocytopenia, neutrophilic leukocytosis, and anemia. Complications such as septic shock, acute kidney injury, and hepatic involvement were observed. All responded to doxycycline within 48 h. No fatalities were observed. Early clinical suspicion of ST among those with high fever, skin rash and thrombocytopenia, and transaminitis showed positive clinical outcome.

Resumen Introducción: Los niveles de citoquinas podrían ser determinantes en la severidad de la Fiebre Dengue. La supresión de su actividad con inmunomoduladores modificaría su evolución. Objetivo: Determinar el efecto de la adición de doxiciclina sobre el perfil de citocinas de pacientes con dengue y valorar la respuesta clínica y el impacto en la severidad de pacientes con Fiebre de Dengue. Materiales y métodos: Estudio aleatorio, doble ciego, placebo controlado con doxiciclina. Ambos grupos recibieron la medicación por 7 días y se controló la evolución clínica, laboratorio y niveles de citoquinas en los días 0, 3 y 7. Resultados: Fueron incluídos en el estudio 61 pacientes, 36 (59%) en el grupo placebo y 25 (41%) en el grupo doxiciclina. Se observó que los pacientes tratados con doxiciclina mostraron menor plaquetopenia en el día 0 y 3 [(170029±47483/mm3 vs 198875±52211/mm3)(p<0,05) y 129545±62840/mm3 vs 165048±51142/mm3)(p<0,05)] al compararlo con el grupo placebo. Los niveles de IL-6, TNF y raIL-1 no mostraron diferencias significativas. Se observó una tendencia a menores niveles de IL-1ß en el grupo tratado con doxiciclina (52,1±97 vs 3,3±7,7)(0,08). Conclusiones: No se constató diferencias en la presentación clínica, hospitalización ni en la mortalidad entre pacientes tratados con doxiciclina comparados con placebo. Hubo una tendencia a menores niveles de IL-1ß en los pacientes tratados con doxiciclina. Debe aumentarse el tamaño de la muestra para confirmar o rechazar los resultados.


Abstract Background: Cytokine levels could be determinant in the severity of Dengue Fever. The suppression of their activity with immunomodulators would modify its evolution. Objective: To determine the effect of doxycycline on the cytokine profile of patients with dengue fever and to assess the clinical response and the impact on the severity of patients with dengue fever. Materials and methods: Randomized, double-blind, placebo-controlled study with doxycycline. Both groups received the medication for 7 days and clinical evolution, laboratory and cytokine levels were monitored on days 0, 3 and 7. Results: 61 patients were included in the study, 36 (59%) in the placebo group and 25 (41%) in the doxycycline group. It was observed that patients treated with doxycycline showed lower thrombocytopenia at day 0 and 3 [(170029±47483/mm3 vs 198875±52211/mm3)(p<0.05) and 129545±62840/mm3 vs 165048±51142/mm3)(p<0.05)] when compared to the placebo group. Lymphatic malformations and its management are not well described in the Dominican Republic. That is why this article's objective is to present 3 cases, with different patterns and treatment needs, so it will work as a reference for healthcare workers in developing countries.

Introdução: A Doença Periodontal tem caráter multifatorial, já que depende de condições microbiológicas, imunogenéticas e sistêmicas do hospedeiro. Representa inflamação crônica das estruturas de suporte e proteção dental. Desencadeia uma complexa estimulação imunológica, bem como a produção de citocinas inflamatórias, que mediam a destruição óssea e de tecido conjuntivo, provocando perda dental e complicações à distância. A compreensão da etiopatogênese, permitiu os conceitos de modulação, que referem-se às modificações dos aspectos danosos da resposta inflamatória. Objetivo: O presente artigo tem como objetivo realizar uma revisão dos estudos sobre as principais terapêuticas adjuvantes na modulação da resposta imune frente à doença periodontal. Revisão de Literatura: Foi realizada uma revisão da literatura, onde foram selecionados artigos científicos em inglês, publicados entre os anos 2005 a 2020, por meio das bases de dados PubMed e ScienceDirect. No decorrer das buscas, foram utilizadas as palavraschaves "Inflamation", "Periodontal Disease", "Subantimicrobial Dose of Doxycycline", "Periodontal Disease", "Host Response Modulation". Resultados e Conclusão: A literatura é bem promissora em relação à terapia de controle complementar da doença periodontal. Dessa forma, novas pesquisas nessa área podem trazer inúmeros beneficos aos pacientes, sendo, assim, um novo caminho para o contorno da resistência bacteriana(AU)


Introduction: Periodontal disease has a multifactorial character, depending on the host's microbiological, immunogenetic and systemic conditions. It represents chronic inflammation of dental support and protection structures. It triggers a complex immune stimulation, as well as the production of inflammatory cytokines, which mediate bone and connective tissue destruction, causing tooth loss and complications at a distance. The understanding of etiopathogenesis allowed the concepts of modulation, which refers to the modifications of the harmful aspects of the inflammatory response. This article has the escape of conducting a review of studies on the main mechanisms of modulation against periodontal disease. Objective: This article aims to rev iew the studies on the main modulation mechanisms in the face of periodontal disease. Literature Review: A literature review was carried out in which scientific articles were selected in English, published between 2005 and 2020, through the PubMed and ScienceDirect databases. During the searches, the keywords "Inflammation", "Periodontal Disease", "Subantimicrobial Dose of Doxycycline", "Periodontal Disease", "Host Response Modulation". Results and Conclusion: The literature is very promising with complementary control therapy for periodontal disease. Thus, new research in this area can bring countless benefits to patients, thus being a new way to bypass bacterial resistance(AU)

ABSTRACT

Purpose: To identify factors other than macular edema and retinitis location responsible for poor visual outcomes in epidemic retinitis (ER). Methods: A retrospective, observational, comparative study. Eyes with corrected distant visual acuity (CDVA) 20/200 or worse at resolution formed Group A. Eyes with central macular thickness (CMT) 600 ?m or worse and retinitis within 1500 ?m to foveal center at the presentation, but improved to CDVA 20/200 or better at the resolution formed Group B. The patient’s history, clinical presentation, imaging, and treatment outcomes were studied and the factors responsible for the final visual outcomes were compared in both groups. Results: Groups A and B included 25 eyes each. The mean CDVA at the presentation was 20/400 (range: 20/125–20000) and 20/320 (range: 20/80–20000), and mean CMT at the presentation was 948.5 ?m (range: 520–1553) and 912.2 ?m (range: 615–1250) in Groups A and B, respectively. All eyes except 1 (Group A) had retinitis lesions within 1500 ?m of foveal center. The mean CDVA at the resolution was 20/400 (range: 20/200–20/20000) and 20/40 (range: 20/20–20/80) in Groups A and B, respectively. Older age, male gender, diabetic status, delayed presentation, poor presenting CDVA, bilaterality, presence of keratic precipitates, disk pallor, retinal thinning, and subfoveal deposits had a statistically significant association, whereas the absence of skin rash, ellipsoid zone loss, negative WIDAL, Weil?Felix test, and delayed doxycycline therapy or use of steroids without doxycycline had a statistically insignificant association with poor visual outcomes. Conclusion: Apart from presenting CMT and location of retinitis, multiple demographic, clinical, and imaging factors can be implicated for poor visual outcomes

ABSTRACT

ABSTRACT Amicrobial Pustulosis of the Folds is a relapsing, chronic and rare neutrophilic dermatosis, characterized by papulopustular, eczematous and aseptic lesions on skin folds. This disorder usually occurs predominantly in females (30 years of age average) with a history of an autoimmune disorder, especially systemic lupus erythematosus. There is no standard therapy, but systemic corticosteroids, alone or in combination with other immunosuppressive drugs, are usually the first-line therapy. We report two females aged 37 and 20 years with the disease but without associated autoimmune diseases. They were successfully treated with non-steroidal treatments.


La pustulosis amicrobiana de los pliegues es una dermatosis neutrofílica crónica, recurrente y poco común. Se caracteriza por lesiones pápulo-pustulosas, eczematosas y asépticas de los pliegues cutáneos. Este cuadro se presenta predominantemente en mujeres de alrededor de 30 años con enfermedades autoinmunes, especialmente lupus eritematoso sistémico. No existe un tratamiento estándar pero los corticoides solos o con inmunosupresores se usan de primera línea. Informamos dos mujeres de 27 y 20 años sin patología autoinmune, con la enfermedad. Ellas fueron tratadas exitosamente sin usar esteroides.

Introduction: The diagnosis of ulcerative colitis is relatively complex because the symptoms are similar to those seen in several other diseases. Objective: To report a case of rectal chlamydial infection whose initial symptoms resembled ulcerative colitis. Case report: A 50-year-old male patient presented with diarrhea, blood and mucus in the stools, and an ulcer in the rectum. The histopathological exam pointed to chronic, unspecified inflammation. After a broad serological screening, with Immunoglobulin M positive for Chlamydia and a high titer of immunoglobulin G, the patient was treated with antibiotics and is clinically cured. Later, he remained Immunoglobulin M positive, but the titers of immunoglobulin G lowered considerably. Chlamydia has been shown to live in the gut microbiota, which could explain the case. Conclusion: It is important to search for chlamydial infection as a differential diagnosis of ulcerative colitis.


Introdução: A retocolite ulcerativa é uma condição clínica de diagnóstico relativamente complexo, uma vez que apresenta sinais e sintomas comuns a muitas outras doenças. Objetivo: Relatar um caso de infecção anorretal por clamídia, cujos sintomas iniciais se pareciam com os de retocolite ulcerativa. Relato de caso: Paciente de 50 anos, do sexo masculino, apresentou-se com diarreia, muco e sangue nas fezes, e úlcera no canal anorretal. O exame histopatológico mostrou um processo inflamatório crônico e inespecífico, então procedeu-se a amplo rastreamento sorológico, que revelou Imunoglobulina M positivo para clamídia e altos títulos de Imunoglobulina G. O paciente foi tratado com antibióticos e encontra-se clinicamente curado. No seguimento, permanece com Imunoglobulina M positivo, mas os títulos de Imunoglobulina G decresceram consideravelmente. Bactérias do gênero Chlamydia têm sido reportadas como parte da microbiota intestinal, o que poderia explicar tal comportamento sorológico. Conclusão: É importante rastrear por clamídia como diagnóstico diferencial das suspeitas de retocolite ulcerativa

Resumen INTRODUCCIÓN: Las úlceras de Lipschütz son una causa infrecuente de úlcera genital, de alivio espontáneo y casi siempre benignas, aunque en algunas pacientes pueden dejar cicatrices genitales. Lo común es que aparezcan en mujeres jóvenes, antes del inicio de la vida sexual activa, en coincidencia con un cuadro catarral o pseudogripal. CASO CLÍNICO: Paciente de 18 años con una úlcera vulvar dolorosa, coincidente con un episodio de amigdalitis aguda y fiebre de 38 ºC. La paciente negó haber tenido relaciones sexuales. La úlcera alcanzó 3 a 4 cm, profunda, purulenta, con apertura del labio menor derecho y con importante componente necrótico. Para el control del dolor se le indicaron: corticoides, doxiciclina oral, crema con lidocaína y antiinflamatorios. Los análisis de laboratorio descartaron que se tratara de infección de trasmisión sexual. Al término del esquema terapéutico prescrito la evolución fue favorable, con desaparición de los síntomas, pero con una secuela: apertura del labio mayor derecho. CONCLUSIONES: La úlcera de Lipschütz es una causa infrecuente de úlcera vulvar. Su tratamiento consiste en el control de los síntomas y casi siempre se cura en el transcurso de 4 a 6 semanas, sin dejar lesiones. El diagnóstico solo puede establecerse luego de excluir otras causas más frecuentes de úlcera vulvar.


Abstract INTRODUCTION: Lipschütz ulcers are an infrequent cause of genital ulcer, of spontaneous relief and almost always benign, although in some patients they may leave genital scars. They usually appear in young women, before the onset of active sexual life, coinciding with a catarrhal or flu-like condition. CLINICAL CASE: 18-year-old female patient with a painful vulvar ulcer, coinciding with an episode of acute tonsillitis and fever of 38 ºC. The patient denied having had sexual intercourse. The ulcer was 3 to 4 cm, deep, purulent, with opening of the right labium minora and with a significant necrotic component. For pain control she was prescribed corticosteroids, oral doxycycline, lidocaine cream and anti-inflammatory drugs. Laboratory tests ruled out sexually transmitted infection. At the end of the prescribed therapeutic scheme the evolution was favorable, with disappearance of symptoms, but with a sequel: opening of the right labium majus. CONCLUSIONS: Lipschütz ulcer is a rare cause of vulvar ulcer. Its treatment consists of symptom control and it almost always heals within 4 to 6 weeks, leaving no lesions. The diagnosis can only be established after other more frequent causes of vulvar ulceration have been excluded.

ABSTRACT

ABSTRACT We report an unusual case of brucellosis presented with headache, diminished vision, papillitis and multiple peripapillary hemorrhages accompanied by subretinal fluid extending up to macula. Diagnosis of brucellosis was made based on positive polymerase chain reaction of cerebrospinal fluid sample for Brucella species DNA, accompanied by a raised titer of anti-brucella antibodies. Patient showed remarkable improvement on triple drug therapy in form of doxycycline, rifampicin and ceftriaxone.


RESUMO Relatamos um caso incomum de brucelose apresentada com cefaleia, visão diminuída, papilite e múltiplas hemorragias peripapilares acompanhadas por fluido sub-retinal, estendendo-se até a mácula. O diagnóstico de brucelose foi feito com base na reação em cadeia da polimerase positiva de amostra de líquido cefalorraquidiano para DNA de espécies de Brucella, acompanhada por um título elevado de anticorpos antibrucela. O paciente apresentou melhora notável com a terapia tripla com drogas na forma de doxiciclina, rifampicina e ceftriaxona.

ABSTRACT

@#Introduction: Earlier attempts to stabilise an emulsion, intended for chronic periodontitis treatment which composed of doxycycline hyclate (DH), Nigella sativa oil (NSO), eugenol and several combinations of surfactants failed. To solve the issue, we investigated the ability of lecithin alone and its combination with hydroxypropyl methylcellulose (HPMC) to stabilise the emulsion. Method: Compatibility between DH and other ingredients was first investigated using DSC and ATR-IR. The emulsion was characterised, firstly by preparing three phases: doxycycline/preservatives with or without HPMC (varying quantities), NSO/eugenol and lecithin/surfactants as aqueous, oil and emulsifier phases, respectively. The phases were added and emulsified sequentially at 7000 rpm (10 min) with an overhead stirrer and then at 3000 rpm (15min) using a high-shear mixer. DH assay was performed using validated HPLC method. Results: All ingredients were found to be compatible with doxycycline based on DSC, ATR-IR and supported by acceptable recovery (98.2±2.2 %) of DH from the emulsion. Stable emulsions were produced with particle size of 198.6±8.2 to 279.3±10.7 nm and zeta potential of -48.2±0.4 to -64.0±3.9 mV. The emulsions showed high viscosity (~200 Pa.s) at zero shear rate and exhibited shear-thinning flow upon increased in shear stress yielding viscosity of ~3 Pa.s at 100 s-1 indicating pseudoplastic behaviour suitable for pre-filled syringe packaging intended for delivery into periodontal pocket. Conclusion: Lecithin is an excellent emulsifier that can also impart pseudoplasticity for a complex emulsion constitute of drug and natural oils. This could pave the way for a more complex emulsion formulation fusing contemporary and therapeutic oils

ABSTRACT

Objective:To analyze the clinical characteristics of pneumonia caused by Chlamydia psittaci (C. psitttaci). Methods:A retrospective analysis was performed on the clinical data of 13 consecutive patients with C. psitttaci pneumonia admitted to the First Affiliated Hospital of Xiamen University from November 2018 to February 2021. Results:All 13 cases had symptoms of fatigue and 6 cases had headache. At consultation, the ΔSequential Organ Failure Assessment (SOFA) scores of all patients were ≥2 points. According to the Pneumonia Severity Index (PSI) score, 2 patients were grade Ⅱ and the other 11 patients were grade Ⅳ or Ⅴ. Laboratory tests showed that C-reactive protein (CRP) and procalcitonin (PCT) levels were elevated in all patients; CRP≥100 mg/L was found in 11 cases and PCT≥0.5 ng/ml was found in 9 cases.There were 12 cases with respiratory failure and 12 cases with elevated transaminase. Chest CT scans showed multiple patchy exudative shadow, focal consolidation and air bronchial sign; and the lesions were mainly in the lower lungs (8 cases). C. psitttaci infections were confirmed by metagenomics next-generation sequencing (mNGS) and the patients′ conditions improved rapidly after timely adjustment of doxycycline based drug treatment and active organ support. The lesions were completely absorbed without residual fibrous cord changes and the prognosis was good. Conclusions:Pneumonia caused by C. psitttaci usually presents sepsis, and the disease progresses rapidly. The mNGS is of value for the early diagnosis of C. psitttaci pneumonia. Timely adjustment of antibiotics treatment after etiological diagnosis can lead to a good prognosis.

OBJECTIVE@#To investigate the expression of matrix metalloproteinase-2 (MMP-2) and matrix metalloproteinase-9 (MMP-9) in multiple myeloma (MM) patients, and analyze the effect of doxycycline (DOX) on the expression of MMP-2 and MMP-9 in MM cells.@*METHODS@#The peripheral blood and bone marrow samples of MM patients were collected, and the patients were divided into three groups: newly diagnosed group, remission group and relapsed/refractory group, while the peripheral blood samples of 34 health people and the bone marrow samples of 17 IDA patients were selected as normal control and control group. The levels of MMP-2 and MMP-9 were detected by ELISA. The protein levels of MMP-2 and MMP-9 in H929 cells treated by different concentrations of DOX were analyzed by Western blot. After H929 cells was treated by Akt inhibitor MK-2206 2HCl in combination with DOX, Western blot was used to detect the levels of MMP-2 and MMP-9.@*RESULTS@#The levels of MMP-2 and MMP-9 in newly diagnosed MM patients were higher than those in control (P<0.05), while for the patients in the remission group were decreased, but still higher than those in control. The levels of MMP-2 and MMP-9 were increased again for the patients in relapsed/refractory group, and showed no significant difference as compared with those in newly diagnosed group. The levels of MMP-2 and MMP-9 could be inhibited by 10 mg/L and 15 mg/L DOX treated by H929 cell. The protein levels of MMP-2 and MMP-9 showed no altered in H929 cells treated by 5 nmol/L MK-2206 2HCl alone. DOX exerted more profound inhibitory effect to MMP-2 and MMP-9 expression in H929 cells when Akt inhibitor MK-2206 2HCl was combined with DOX.@*CONCLUSION@#The levels of MMP-2 and MMP-9 are increased in MM patients and related to the disease status of MM. DOX can inhibit the expression of MMP-2 and MMP-9 in MM cells, and antagonizing its activation of Akt signaling pathway can further enhance the inhibitory effect.

OBJECTIVE@#To investigate the mechanism of the in vitro toxicity of doxycycline to myeloma cell line H929 and also the possible pathway involved its toxicity.@*METHODS@#Myeloma cell line H929 was treated with DOX, MEK inhibitor U0126 or RAS agonist ML-098, either alone or in combination. Then, the expression of p-MEK, caspase-3, caspase-9 and c-Jun in H929 were used to detected by Western blot; the cells proliferation and apoptosis were detected by CCK-8 assay and flow cytometry, respectively.@*RESULTS@#DOX significantly increased the levels of cleaved caspase-3 and caspase-9, and down-regulated the level of p-MEK in H929 (P<0.05). MEK antagonist U0126 significantly increased the levels of cleaved caspase-3 and caspase-9, and down-regulated the level of p-MEK (P<0.05). After Dox combined with ML-098 treatment of H929 cells, the apoptosis rate of H929 cells was lower than that of DOX alone treatment group(P<0.05). Compared with DOX alone treatment group, the expressions of p-MEK and p-ERK1/2 in DOX+ML-098 combined treatment group were increased, and the levels of cleaved caspase-3,9 in H929 cells were decreased (P<0.05). The levels of c-Jun mRNA and protein increased in H929 when treated by DOX alone (P<0.05).@*CONCLUSION@#DOX can induce apoptosis of H929 via intrinsic apoptosis pathway, and MEK/ERK pathway and c-Jun possibly play a role in this process.

ABSTRACT

Objective:To investigate outcomes and safety of doxycycline-moxifloxacin sequential regimen in the treatment of Mycoplasma genitalium urethritis/cervicitis. Methods:From June 2019 to December 2020, patients with Mycoplasma genitalium urethritis/cervicitis confirmed by nucleic acid amplification testing were successively recruited at Department of Sexually Transmitted Diseases, Hospital of Dermatology, Chinese Academy of Medical Sciences, and received sequential therapy with oral doxycycline for 7 days followed by oral moxifloxacin for 7 days. Clinical and/or etiological assessment was conducted 2 to 3 weeks after the end of treatment. Fisher′s exact test was used to analyze factors influencing the treatment outcome. Results:Totally, 36 eligible subjects were enrolled, including 30 males and 6 females. Among them, 18 (50%) patients completed post-treatment etiological assessment, which showed that 12 achieved microbiological cure, and treatment failures occurred in 6; another 18 patients achieved clinical cure. The overall response rate to doxycycline-moxifloacin sequential therapy was 83.3% (30/36, 95% confidence interval[ CI]: 70.5%, 96.1%) . The treatment outcome showed no significant association with the patients′ age, gender, marital status, number of sexual partners in the past 1 month, history of sexually transmitted diseases, history of antibiotic use in the past 1 month, or co-infections (all P > 0.05) . Conclusion:The efficacy of doxycycline-moxifloacin sequential regimen is limited in the treatment of Mycoplasma genitalium infections in Nanjing area, and clinicians should be alerted to the possibility of treatment failure in clinical practice.

Background:Periodontal instrumentation is accessible to a certain limit, as subgingival calculus is difficult to remove completely in cases of deep pockets and furcation defects. Host modulating agents aim to inhibit arachidonic acid pathways, modulate MMPs, bone remodeling, and regulate immune and inflammatory responses. This systematic review aimsto find the effects of Low Dose Doxycycline (LDD) as local drug delivery systems and sub-antimicrobial dose doxycycline for the treatment of periodontal disease.Material And Methods:Electronic database searched were: Pubmed, Medline, Scopous and Ebsco was performed using MeSH terms: low dose doxycycline, subantimicrobial dose doxycycline, chemically modified doxycycline, doxycycline, doxycycline hyclate, atridox and periostat. Articles published between years 2008-2019 were reviewed. Result:A Systematic review methodology was followedand database searching was done which yields 422 records. Records from year 2008 –2019 were taken into consideration. After inclusion and exclusion accordingly, A total of 10 studies were systematically reviewed. Non surgical or surgical periodontal therapy is more effective in reducing periodontal disease when low dose doxycycline is used as an adjunct either locally or systematically delivered. Conclusion:Subantimicrobial Dose Doxycycline (SDD) is commonly accepted as a host response modifier and it Subantimicrobial Dose Doxycycline (SDD) which down regulates the activity of MMPs. Also, doxycycline provides long-term post-treatment effects. Based in the inference drawn from this systematic review, non surgical or surgical periodontal therapy is more effective in reducing periodontal disease when low dose doxycycline is used as an adjunct, delivered either locally or systematically.
